Our goal is to provide care coordination and access to comprehensive wrap-around services which engage participants in recovery-oriented discussions and improve their vital conditions.
Currently, we are focusing on assisting participants in obtaining employment, accessing substance use and mental health treatment, achieving financial wellness, improving physical health by linking participants with primary care providers and specialists and ultimately achieving and keeping permanent housing.
